From August 2023 – September 2023, SMC Health’s Public Health, Policy, and Planning (PHPP) will be conducting a Community Health Improvement Plan (CHIP). The CHIP will engage various community groups to identify health improvement priorities. The health priorities will be informed by data highlights from a recently completed Community Health Needs Assessment.
