Local Health Departments Convene in Santa Rosa
In October, SMC Health staff presented at the annual County Health Executives Association of California (CHEAC) meeting to share about work to address Food and Nutrition Insecurity among Medi-Cal patients. The presentation described partnerships between the San Mateo Medical Center, the Health Plan of San Mateo, Ravenswood Family Health Clinic, and San Mateo County Public Health Policy and Planning, to link healthy food access to healthcare.
San Mateo County partners are working to create a system of care that provides sustainable and accessible food resources to patients in need through the implementation of CalAIM. CalAIM provides Community Supports to address patients’ health related social needs from housing support to asthma remediation to food. Medically Supportive Food and Nutrition interventions provide nutrient rich whole food, to prevent, treat or reverse diet-sensitive health conditions and chronic disease.
