Funding Available for the San Mateo County Community Health Improvement Plan Collaborative!
Public Health, Policy & Planning is now accepting proposals for the San Mateo County Community Health Improvement Plan (CHIP) Collaborative. The RFP seeks proposals that will cover two years of services and will not exceed $400,000 in total. The CHIP serves as a blueprint for shared action. It works to advance community priorities of Access to Health Care Services, Mental Health, and Social Determinants of Health. The RFP seeks to fund one qualified vendor to lead a sustainable structure for community collaboration to serve as the CHIP Collaborative.
