2025-2027 Get Healthy San Mateo County Community Implementation Funding Grantees

Three projects were selected to receive the 2025 – 2027 Get Healthy San Mateo County Community Implementation Funding to strengthen healthy and equitable communities for all who live, learn, work, and play in San Mateo County. The funding furthers community-identified priorities in alignment with the 2024 – 2026 Community Health Improvement Plan. See below or for the full description of the projects and new awardees.

Priority Area Grantee/Project

Access to Health Care Services

  • Expanding access to health services is critical to improving health outcomes and reducing disparities
  • Project to increase the proportion of SMC community members who can easily access quality health care services that meet their holistic needs, especially individuals and communities who are experiencing barriers caused by structural inequities including, but not limited to, racism, sexism, classicism, ageism, and homophobia
Anamatangi Polynesian Voices will build a Community Health Network that aligns partners to deliver mental health and peer support programs. It aims to increase access to culturally-responsive health care in East Palo Alto.

Social Determinants of Health (SDOH)

  • SDOH are the conditions in which people live, learn, work, and play that have a major impact on health, well-being, and quality of life
  • Project to improve coordination among organizations providing essential services across sectors and to advance policies that improve SDOH for those experiencing disparities
Fresh Approach will expand Food as Medicine under CalAIM with the local Managed Care Plans. The project addresses food and nutrition insecurity by onboarding community-nutrition providers, setting up a referral and reimbursement method, and closing the loop for patients to receive healthy food to improve their long-term health outcomes.

Mental Health

  • Mental health is an important aspect of overall health and well-being
  • Project to develop a county-wide and cohesive approach to improve mental well-being with a focus on decreasing disparities, especially among individuals and communities who are experiencing barriers caused by structural inequities
Redwood City School District will establish a wellness counselor to serve 12 elementary and middle schools. This will expand mental health care coordination, parent workshops, and counseling services. This project also supports new families particularly immigrants, refugees and youth at risk of homelessness.
